How I did it: I downloaded the renewal form in PDF from the State Department web site, printed it out, filled it out, and sent it in with my old passport, a check and two passport photos. I ordered expedited service because I hate being without a passport.
Lessons & tips: The interiors of the new U.S. passports are a bit over-the-top in their imagery. They also contain RFID chips. If you think that you might not want just anybody to be able to detect your passport (and potentially to decrypt your personal details stored therein), you may want to look into an RFID-blocking wallet to store it in.
Resources: I ordered an RFID-blocking passport case from DIFRwear. There are a number of companies out there making similar products. I chose DIFRwear because a privacy hacker site tested their product.
