How I did it: My friend talked me into getting my nipples pierced.  They'd always been something I'd barely considered because I was not much interested.  But within a week of her telling me I should; I got them.  The idea just grew on me in those 7 days.  Worst pain I've ever felt, but it lasted 2 seconds per nipple and it was DONE.  OVER.  Just as you're realizing, "crap this is going to hurt" it's already over.  They healed amazingly well with H2Ocean (I highly recommend this stuff for every piercing) and I could not be happier with them!  The piercer I went to was so professional and made me feel really calm about the situation.

Lessons & tips: Captive bead rings (CBRs) provide a good way to clean your piercing, but barbells are going to be more comfortable.  For women I recommend sports bras.  Use Ivory bar soap to clean twice a day, and use H2Ocean to your heart's content.  I did not encounter one problem while healing (except that CBRs definitely got knocked around a lot).
