How I did it: I got the form at the post office (the online form wasn't working when I tried to fill it out), and completed all of the information. Then I picked a day and went to the post office, where they took my picture, my birth certificate, and I paid them the money.
Lessons & tips: Have an original certified copy of your birth certificate. You may have to send off for this. Get your parents' birthday and SSN for the form. Have enough money to pay the fees.
