How I did it: Found a doctor who really listened and took the time to figure out what diet/exercise approach was physiologically correct for me.
Figured out that the social aspect of exercise was an important motivator for me.
Figured out that I like tennis and swimming so much, it doesn't seem like exercise, so I willingly do them regularly.
Got a WiiFit and use it regularly.
Lessons & tips: Make it part of your every day, like brushing your teeth!
Find what you like, so you'll want to do it, even if this means constantly changing what you do. Just keep moving and trying new things.
Make it a social thing, so if you don't go, someone will miss you.
