help my mother change her life for the best

My mother is on government support, she hasn’t worked for the last 20 or more years, she is addicted to smoking and is an alcholic. She has many health because of this and other things and if she doesn’t stop, she will die. She is moving out to my hometown and I want it to be a new start for her, I want her to clean up, grow up, start working, and give herself and my brother and sister a better life.
