I received the forms through an email attachment from Vanguard. 19 months ago

There are two of them. I now have to:

(1) Fill them out.
(2) Take them to a bank and have them certified.
(3) Copy them and mail the originals.

Unfortunately, there are no brick-and-mortar offices of Vanguard. “But our customer service reps are standing by 24-hours-a-day to help you if need be.” Not helpful.

I sent an email to the broker... 19 months ago

and asked (1) What forms do I need, and (2) do you have a brick-and-mortar so I could walk in and get this done?

I also found the trust docs, and added the account numbers from my brokerage accounts.
