This has been languishing on my list for a while now, so I’m adding a little more information.
The following is required for a passport:
1) A certified copy of my birth certificate. I only have one, and I’m not comfortable giving the one I have to the feds to lose, so I’ll have to send a check and request to this place: http://www.dhss.mo.gov/BirthAndDeathRecords/index.html
2) Two identical passport photos—which I can apparently take care of when I turn in my application at Sacramento City Hall: http://www.cityofsacramento.org/clerk/passport/
3) $100 (!) for the book.
So, I will mail the birth certificate request tonight and schedule the city hall appointment as soon as I receive it.
