Howard Richman’s Super Sight-reading Secrets is the best instruction book I’ve found on this subject. According to Howard it’s going to take anywhere between six months and four years to reach a level advanced enough to play hard pieces directly from the paper, if you follow his instructions.

I’ve just begun using the method and I intend to stick to it until I’ve reached that level.
