I am trying to set up a scholarship for my daughters school band for parents who can afford to pay the full amount of the fees. I have the account set up and have started receiving the money. But I do not know how to set a form for the parents to apply for the scholarship. Does anyone have an example of how the form should look like and what questions I should ask? I would appreciate any help I can get. Thanks
