Try Mensa.org. They have resources for finding accreditated testing organizations. They also have a mail in test to see if you are a good candidate for taking the offical MENSA test.

Is this an actual test? Or am I just posting on the internet the fact that I want to take such a test? Umm…right. Well.
I did, recently, take one online—and got a 150. But I was looking for an “official” one, so’s I can actually say, “Yeah, this is my score, thanks.”
But anyway. Whichever.

tests that are considered ‘official’ or ‘PhD approved’. It seems my intelligence ranges from slightly above average to lower-end genius. More or less though I think, about mentally averaging them out, I think I’m around 120 or so…
